Move PTR Player RBAC grants to mod-gm-commands

The auth-DB SQL added in earlier commits on this branch is being
consolidated into mod-gm-commands so operators have a single source of
truth for chromiecraft RBAC role definitions. See:

  chromiecraft/mod-gm-commands#11

This branch now strictly removes the obsolete world-DB
cc_ptr_commands.sql (silent no-op under PR #24641); the RBAC grants
that replaced it live in the gm-commands module.

Effect of this PR alone: deletes the dead world-DB file. Effect when
combined with the gm-commands PR: PTR Player role 1000 is created
there, auto-granted to all sec-0 accounts via role 199, with the same
131 command perms previously defined in this branch.

Pre-requisite: install mod-gm-commands. If a realm runs mod-ptr-changes
without mod-gm-commands after this change, sec-0 accounts will lose the
PTR-tester command access. That trade-off is intentional -- duplicating
the role definitions in two modules creates maintenance drift.
